Coronavirus 19/COVID-19 Vaccination Information

The Sacred Peaks Health Center is hosting a Drive-thru COVID-19 Vaccine event for adults aged 65 years and older.

Who can get a COVID-19 vaccine at this event?

  • Must be 65 years old or older
  • Priority given to patients receiving primary care at Sacred Peaks Health Center, Flagstaff, AZ

What to prepare and expect?

  • If possible, please have a driver and interpreter.
  • Download and complete COVID-19 Vaccine Form
    • Pickup Packet:  Sacred Peaks Health Center

* A date and time will be determined for the 2nd vaccine (booster) dose at Sacred Peaks Health Center in Flagstaff, AZ

Can you get a COVID-19 Testat SPHC during the event?

No. The Sacred Peaks Health Center COVID-19 Triage Tent will be closed during the event - January 22, 2021. The Coconino County Health Department COVID-19 testing will be available.

Do not show up if you:

  • Have a cough, fever, shortness of breath, or loss of taste or smell or any other illness.
  • Had COVID-19 in the past 3 months (after OCT 2020)
  • Are in quarantine for a household member with COVID-19 or an exposure.
  • Have a pending COVID-19 test.
  • Had any other vaccination in the past 14 days.
