Coronavirus 19/COVID-19 Vaccination Information

The Tuba City Regional Health Care Corporation (TCRHCC) is hosting a Drive-thru COVID-19 Vaccine event for adults aged 65 years and older.

Friday, January 15, 2021
9:00 AM to 3:00 PM
Tuba City High School - Pavilion parking lot.

Who can get a COVID-19 vaccine at this event?

  • Must be 65 years old or older
  • Must have a Patient Medical Record established at TCRHCC, Sacred Peaks Health Center, or LeChee Health Facility
  • Must be living in the TCRHCC Service Unit

What to prepare and expect?

  • If possible, please have a driver and interpreter.
  • Download and complete COVID-19 Vaccine Form
    • Pickup Packet: Local Chapter House (TCRHCC Service Unit)
  • Must commit to return for 2nd vaccine dose on Friday, February 5, 2021 from 9:00 AM to 3:00 PM at Tuba City High School- Pavilion Parking Lot

Do not show up if you:

  • Have a cough, fever, shortness of breath, or loss of taste or smell or any other illness.
  • Had COVID-19 in the past 3 months (after OCT 2020).
  • Are in quarantine for a household member with COVID-19 or an exposure.
  • Have a pending COVID 19 test.
  • Had any other vaccination in the past 14 days.
